Curana Health and Ciena Healthcare are expanding their collaboration to introduce an Institutional Special Needs Plan (I-SNP) model to residents in Ciena’s 31 facilities across Michigan, US.

Through Align Senior Care, a subsidiary of Curana Health, the partnership will provide enhanced benefits and onsite preventative clinical care for Medicare-eligible residents within Ciena Healthcare communities.

The I-SNP is a Medicare Advantage plan tailored for individuals requiring long-term care in senior living and skilled nursing facilities, offering a coordinated approach to care.

Developed with community input, Align Senior Care’s I-SNP focuses on personalised care teams, and in-person clinical rounding, and aims to reduce hospitalisations.

According to a press statement, the model has shown a 39% average reduction in 30-day hospital readmission rates for Medicare Advantage I-SNP members.
